How do I cancel my order and get a refund on device

I paid for the LG Optimus F3 40 days ago. As of now, my device is still being 'kitted' - and I've had no access to a telephone for close to 30 days - and the customer service I've received has been shambolic. I want to go back to my old service provider so as to be able to communicate as this is really frustrating.

I would be grateful if someone can help me with steps to follow. Many thanks.
