HELP! Can't Make Cell Call -- Msg: Number is Incorrect

I received an LG Volt from FP a couple of days ago. I'm able to make outgoing calls on WiFi but when trying to call on 3G or 4GLTE (with wifi turned off) I always get a msg that says the "The number or code you dialed is incorrect." I tried calling multiple phones, both cell and landline. I have updated PRL, updated Profile, Updated Firmware, Updated LG Software. No help.

The only app that I've added is Google Voice. GV is set to "Do not use GV to make any calls". I do receive texts and calls that are made to my Google Voice phone number successfully.

I have 826MB of Data remaining.

How do I fix this?

    I have also uninstalled the FP Messaging App and then reinstalled it. No improvement.
