Carrier Reset did not work on HTC Evo 4G - Still getting Error Code 128

I got the Device Notification a week ago (5/3), just after I started to not be able to use my phone. Since then, I have still not been able to connect my phone to the cellular network. I have done multiple carrier resets and got nothing following other than an Error Code 128 (Unable to establish wireless data connection.)

Please advise.
