Please help: Unable to access mobile data now

I upgraded to the Premium service package, and I was sent an email instructing me to perform a SCRTN, which I did. Now, my Samsung Galaxy S4, running lollipop, no longer has 3G or 4G access, and a hands-free activation screen runs every time I power-cycle the phone, failing to activate every time. Any advice is appreciated.

    OK, it now connects to the 4G network, but only outgoing phone calls work; all incoming phone calls, either on WiFi or mobile data, return an error message to the caller.
