New phone arrived without a SIM card

I loaded ticket 180906-001534. Took 13 days to get iPhone 5 from the special sale. Turn it on and it says No SIM Card in upper left corner. Pop open door and no SIM card. Really? How did you configure my selected phone number without a SIM. Please send proper SIM ASAP or refund my purchase immediately.
