How to get refund back from freedompop

I had opted in for the free SIM offer on 5th September 2018 but the same day cancelled my account. I never activated the SIM card given by Freedompop. I was charged $20 as deposit for any "excess" changes on the free plan but after cancelling the account more than a week ago, I have not received any refund of the deposit.

I tried calling freedompop customer support number but that is useless since I am required to have my Freedompop account number which I don't have as I have closed my account.

What does it take a customer to get his refund back from Freedompop ?
