Phone stopped working for calls/SMS, and service now says "Inactive"

My phone stopped working for calls and texts a few days ago. We updated, reset using ##72786# (Android), and still nothing. Going into our account, it shows inactive, even though we paid for a year just four months ago (my credit card # has since changed, but there should be no charge to that card, since we paid for a year's service, so I would not expect there to be a problem with CC info).

Additionally, somehow my name has changed on my account (it's close, but has a few extra letters!).
