12-month plan and GSM SIM kit activation problem

I have just purchased a 12-month plan and GSM SIM kit (1 GB per month) from Amazon.com. When I went to activate the SIM at www.freedompop.com/activate, it only gave me monthly plans choices (Premium 2GB/4GB/Basic) instead of the prepaid 12-month plan that I paid for. Could someone please fix the SIM (ICCID: 89011704278341521419) so I can activate the prepaid 12-month plan? Thank you very much.
