Why I've been still charged $9.99 every month after canceling FreedomPop Premier

Hi,

I canceled FreedomPop Premier last December then I received the email "Your FreedomPop Premier Service Has Been Cancelled". But I am still charged $9.99 every month.
I don't know why I am charged, it is definitely mistake and system error.
How can I get a $40 refund?

http://www.freedompop.com/acct_services.htm shows:
"You have not signed up for any services yet.
Check out the available services below to get the most out of your FreedomPop Free 500MB 4G plan."
