Device Swap Request

I would like a FreedomPop Admin to assist in a device swap request. The devices associated with this swap request are indeed currently active. The two devices involved are my old LG G2 LS980 and my new Samsung Galaxy S5.
I need the new S5 to be put on the old account with my annual plan and services and old phone #, and the new account closed. Please help.
